Enrollment Information
Enrollment Windows:
Enrollment occurs via individual consultation and intake and is by APPOINTMENT only with the exception of the first week of school.
Enrollment appointments are scheduled during the Term Enrollment Windows.
School Year 2012-2013 Enrollment for Term III and Term IV are now closed.
Please continue to visit this website for information on the next open enrollment period.
The following documents are required for enrollment:
1)All students seeking to enroll MUST attend appointment with the parent/legal guardian present: A photo ID with a valid Chicago address is required. Legal guardianship MUST be established by means of documentation. Unestablished or unauthorized persons will not be allowed to enroll students.
2) Proof of Pregnancy/Parenting: Medical statement from doctor/clinic verifying the pregnancy OR valid birth certificate of the baby.
3) Current & updated transcripts and school records from ALL schools previously attended. The students grade level will be determined by the number of credits she has earned regardless of age.
4)Updated medical records and immunization records
